This is an excerpt from a graded piece of work, shown with the marker’s comments so you can see what strong work looks like at this level.
Excerpt: Nursing diagnosis 1
Nursing diagnosis: Risk for unstable blood glucose level related to insulin therapy and irregular meal timing.
Goal: The patient will maintain fasting glucose between 80 and 130 mg/dL for the next 72 hours and will state two signs of hypoglycaemia before discharge.
Intervention: Check glucose before meals and at bedtime, and give rapid-acting insulin within 15 minutes of the meal tray arriving. Rationale: matching insulin to carbohydrate intake lowers the risk of both hypoglycaemia and post-meal spikes (American Diabetes Association, 2024).
Evaluation: Fasting glucose was 112 mg/dL on day two. The patient correctly named shakiness and sweating as warning signs and demonstrated the sick-day rules using the teach-back method.
Marker feedback
Clear prioritisation of nursing diagnoses and strong use of current evidence. Rationales could be shorter.
